Understanding Extreme Hunger in Eating Disorder Recovery

If you’ve ever felt constant physical and mental hunger after a period of food restriction, you’re not alone. This experience, often called post-restriction hyperphagia or extreme hunger, is your body’s way of trying to restore essential tissues, repair metabolic functions, and rebuild trust that food will be available consistently.


Why Does Extreme Hunger Happen?


When you’ve been underfeeding your body—whether intentionally or unintentionally—your biological systems adapt to survive. Metabolism slows, energy is conserved, and certain bodily functions may be suppressed. When you begin nourishing yourself properly again, your body recognizes the opportunity to repair and replenish. This can lead to intense hunger cues and a strong drive to eat, which can feel overwhelming.


Common Fears Around Extreme Hunger


Many of my clients worry that their heightened hunger will never subside or that they’ll only crave high-energy foods forever. I had these same fears during my own recovery. This fear can be exacerbated by diet culture, which often labels certain foods as “bad” or “too much.” However, this is simply your body doing what it needs to do to heal—and it won’t last forever.


My Personal Experience with Extreme Hunger


I understand firsthand how unsettling this can feel. During my own recovery, I remember going months without craving vegetables. It wasn’t that I disliked them; my body just had more urgent needs to address first. The idea of eating non-starchy vegetables felt repulsive for a while, but over time, my cravings naturally balanced out. I also felt as if my physical and mental hunger were not aligned. In other words, I had to eat past physical fullness before mental hunger subsided. With time, I no longer had to eat past physical fullness to feel comfortably satisfied.


Trusting the Process


It’s important to remind yourself that extreme hunger is temporary. Your body isn’t broken—it’s simply trying to find equilibrium. Over time, as your body learns that food is abundant and consistently available, your hunger cues will regulate. You’ll start craving a wide variety of foods like fruits and vegetables, naturally.


What to Do If You’re Experiencing Extreme Hunger


  • Listen to your body – Honor your hunger and cravings instead of fighting them. I know this is scary but extreme hunger happens as part of the healing process. If you try to suppress mental or physical hunger, lack of accurate hunger and fullness cues will be prolonged. 

  • Avoid restriction – Experiencing uncomfortable fullness to feel mentally satisfied is part of the healing process, and it won’t last forever. If you’re craving specific foods you restricted in the past, don’t be afraid to honor this. It may feel like you constantly crave the same foods, but with time, your body will begin to crave a wider variety.

  • Stay patient – Your body is healing, and that takes time.

  • Seek support – Working with a dietitian who understands eating disorder recovery can provide reassurance and guidance.


If you’re navigating this stage of recovery, know that you’re not alone. Healing your relationship with food takes time, but your body knows what it’s doing. Trust the process, and in time, your hunger will find its natural rhythm again.
